Why Interoperability Still Fails

Why Interoperability Still Fails

Interoperability in healthcare exists. But in post-acute and long-term care, it is still failing where it matters most.
Over the past decade, hospitals advanced rapidly with federal investment and clear incentives. PALTC was excluded. The result is a sector managing some of the most medically complex patients in healthcare with some of the least connected systems.
Nearly 70% of hospital-to-SNF transfers include medication discrepancies. Only 16% of hospitals send summary of care records to most or all long-term care providers. And only 8% routinely receive data back.
The problem is not that technology is unavailable. It is that technology, workflow, and policy have never been aligned around how care actually works in these settings.

Recent updates from the Centers for Medicare & Medicaid Services (CMS) are reshaping compliance and reimbursement processes for Post-Acute Long-Term Care (PALTC) providers. While most claims are now processing normally, certain telehealth services remain on hold pending clarification. This article outlines key policy changes, their operational impact on PALTC practices, and how integrated EHR and RCM systems help providers maintain compliance and financial stability.
